How to Estimate Your Take-Home Pay: Understanding the Average Doctor Salary Australia

If you are a doctor or medical professional in Australia, you know that the phrase average doctor salary Australia covers a huge range. An intern’s pay under the Fair Work award is very different from a neurosurgeon’s taxable income. This guide helps you work out what actually lands in your bank account after income tax and the Medicare levy.

What the Data Shows

The Fair Work Ombudsman publishes the Medical Practitioners Award, which sets minimum rates for employed doctors. From 1 July 2026, an intern earns $1,277.54 per week ($33.62 per hour). A resident medical officer at pay point 1 earns $1,356.33 per week. Registrars range from $1,543.23 to $1,731.12 per week depending on experience. Senior registrars and career medical practitioners earn more, up to $2,458.04 per week for a senior career medical practitioner at pay point 3.

The Australian Taxation Office provides a different picture: taxable income for the 2023–24 year. For example, general practitioners averaged $196,060 (median $156,733), while neurosurgeons averaged $678,611 (median $537,049). Emergency medicine specialists averaged $347,306. These figures include all income from salary, private practice, and allowances.

Estimating Your Take-Home Pay

To estimate what you keep after tax, start with your gross annual salary. For a doctor earning $150,000 per year (roughly a senior registrar or early specialist), the 2026–27 tax calculation is:

  • Tax on first $18,200: $0
  • Tax on next $26,800 ($18,201–$45,000): $4,020
  • Tax on next $90,000 ($45,001–$135,000): $27,000
  • Tax on remaining $15,000 ($135,001–$150,000): $5,550
  • Total tax before offsets: $36,570

Then add the Medicare levy: 2% of $150,000 = $3,000 (full rate applies as income is above the $35,013 single threshold).

You may be eligible for the Low Income Tax Offset (LITO). At $150,000, the offset is $325 minus 1.5c per $1 over $45,000, which reduces to $0 by $66,667. So no LITO at this income.

Total deductions: $36,570 + $3,000 = $39,570. Take-home pay: $150,000 – $39,570 = $110,430 per year, or about $9,202 per month.

For a doctor earning $300,000 (e.g., a specialist in private practice), the tax is higher. The top bracket of 45c applies above $190,000. Medicare levy is $6,000. Take-home would be roughly $186,000 per year.

Remember: superannuation is paid by your employer at 12% (from 1 July 2026) on top of your salary, not deducted from it. So your take-home is based on your gross salary, not including super.

Using the Award Rates for Junior Doctors

If you are a doctor in training, your pay is set by the award. Use the weekly rates to calculate annual pay: multiply by 52. For an intern: $1,277.54 × 52 = $66,432.08 per year. Then apply the tax and Medicare as above. At this income, you would pay little tax (under $18,200 tax-free) and the Medicare levy may be reduced or nil if your income is below the $28,011 threshold.

For a registrar at pay point 4: $1,731.12 × 52 = $90,018.24. Tax on that would be about $16,000 plus Medicare $1,800, leaving around $72,200 take-home.

Key Considerations

  • The ATO data is for 2023–24, not current year. Use it as a guide, not a prediction.
  • The Medicare levy surcharge (if you don’t have private hospital cover) is not included here.
  • Salary packaging (e.g., for cars or laptops) can reduce taxable income.
  • HELP/HECS repayments are additional deductions.

Questions

What is the average doctor salary in Australia?

There is no single figure. The Australian Taxation Office reports average taxable incomes for 2023–24 ranging from $140,129 for resident medical officers to $678,611 for neurosurgeons. General practitioners average $196,060. Fair Work award rates for doctors in training start at $1,277.54 per week for interns.

How do I calculate my take-home pay as a doctor?

Start with your gross annual salary. Apply the 2026–27 resident tax rates: nil on the first $18,200, then 15c per $1 up to $45,000, 30c up to $135,000, 37c up to $190,000, and 45c above. Subtract the Medicare levy (2% of taxable income, with thresholds of $28,011 and $35,013 for singles). You may also be eligible for the Low Income Tax Offset. Superannuation (12% from 1 July 2026) is usually paid on top of your salary and does not reduce take-home pay.

Do the Fair Work award rates apply to all doctors?

The Medical Practitioners Award covers most employed doctors in training and career medical practitioners in hospitals. Specialists in private practice are often not covered by the award; their income is determined by contracts and Medicare billings. The ATO data reflects total taxable income from all sources for each specialty.
